TMDA (http://software.libertine.org/tmda/) is immune to all of those
spammer adaptations. Incoming messages that aren't from a known sender
are quarantined until the sender confirms their message. Since almost
all spam has a bogus sender, they never receive the confirmation
request. If a spammer did happen to use a replyable address, the
victim could simply add that address to their blacklist.
