The standard (DSM-IV) definition of delusions is "erroneous beliefs that
usually involve a misinterpretation of perceptions or experiences".
Delusions are operationalised as beliefs that are (for the individual)
indistinguishable from reality (whatever that is). Of course, at different
times (e.g. on treatment) an individual may recognise previous beliefs as
delusional. There's also the more general problem that it's not always
possible to determine empirically whether a belief is false.
