Peltiers are not good for high-power applications, conventional
Carnot-cycle refrigerators are still superior there. Carotid surgery in
the field? Much too invasive. A portable PLV cooling system is much
preferable, because only requiring minimal skills (but people have been
known to even botch up intubations, if you can imagine such a thing).
