If I schlepped over to UCSD medical library and photocopied it,scanned it,
then posted it to this list should everyone who read it send $17.50 to the
publisher?

Could you please explain to me what's unfortunate about a free market
working? You should be ashamed of asking for a password, the publisher has
a right to profit from their information, and what you propose is
rationalized theft (unethical).
