The cause of true canker sores (recurrent aphthous stomatitis) is
unknown. However, the current Merck Manual states that "several
factors point toward a localized immune reaction". Stress and
local trauma are listed as the usual precipitating factors.
Susceptibility is increased with deficiencies of iron, vitamin B12,
and folic acid.
Primary and recurrent herpetic lesions can appear concurrently, as well
as alone in the oral cavity, and are differentiated from canker sores,
as well as, benign mucosal pemphigoid, erythema multiforme, herpangina,
and oral pemphigus by the examining physician.
